Software projects rarely fail because of one dramatic event. They slip through a series of small, predictable risks nobody owned. PMP-style risk management makes those risks visible early, while they are still cheap to address.
Identify risks with the whole team
Run a short workshop at kickoff and revisit it every sprint. Useful prompts:
- Requirements — what is still vague, contested or dependent on someone outside the team?
- Technology — new frameworks, third-party APIs, data migrations, performance targets?
- People — single points of knowledge, availability, onboarding?
- External — app store reviews, compliance approvals, vendor timelines?
Analyse: probability × impact
Score each risk from 1 to 5 for probability and impact and multiply. Anything scoring 12 or more gets an owner and a response plan this week. Keep it simple — consistency matters more than precision.
Choose a response strategy
For threats, PMP describes five responses:
- Avoid — change the plan to remove the risk (for example, use a proven library instead of building one).
- Mitigate — reduce probability or impact (spike the risky integration in sprint one).
- Transfer — shift it to a better-placed party (a managed AWS service or a vendor SLA).
- Escalate — hand it to the level that can actually decide.
- Accept — acknowledge it, and set aside contingency time or budget.
A risk register you can copy
For each risk, record:
- ID and short description
- Category (requirements, technology, people, external)
- Probability, impact and score
- Owner
- Response strategy and next action
- Trigger — the early warning sign that the risk is materialising
- Status
Make it part of the rhythm
Review the top five risks in every sprint planning session and include them in stakeholder updates. When a trigger fires, act on the response plan immediately rather than waiting for the next status meeting.
Don't forget opportunities
Risk management also covers positive risks — a reusable component, an early API release from a partner. Exploit, enhance or share them deliberately.
